Important Notice
All forms must be printed and hand-signed. Digital signatures are not acceptable. If you do not have access to a printer, you can pick up the forms in the Office of Financial Aid, or we can mail them to you upon request.

Appeal Forms
- Special Circumstance Appeal Form (marital status changes, 2022 or 2023 income adjustments)
- Income Estimation Form (optional: complete this form if using 2024 estimated income)
- College Expense Form (optional: complete this form to account for other household member’s college expenses)
- Unusual Circumstance Appeal Form (dependency override, unable to contact parents)
- Cost of Attendance Appeal Form
Non-Appeal Forms
- Parental Refusal Form – parent refuses to complete the FAFSA
- Additional Unsubsidized Loan Form – parent is denied a PLUS loan and you wish to borrow additional funds
- Living Allowance Adjustment Form – switch your financial aid package between living on-campus, off-campus, or with parent
